The total cost for this Reduction Weight Management Program
is $2,500.00 and is not billable to your insurance company.
The $2500 program fee is due in full at the time of your initial physician
consultation. If YOU decide to cancel your surgery for any reason, within two
weeks prior to your operative date, half of the program fee will be refunded.
If YOU cancel your surgery, within one week prior to your scheduled operative
date, there will not be a refund of the program fee.
The program fee does not include your office visits, co-pays, deductibles,
co-insurance, medical care or procedures, protein supplements, or individual
personal training sessions. There will be a fee for utilization of the fitness
center beyond the first year.
